To Our Individual Clients
Your privacy is important to us, and maintaining your trust and confidence is one of our highest priorities. We respect your right to keep your personal information confidential and understand your desire to avoid unwanted solicitations. Providing personal information is an act of trust and it is taken seriously. Unless given consent to do otherwise, FOUNDATION will only collect and use personal information as set out below. We are disclosing this policy to you so that you will have a better understanding of what we do with the information you provide us and how we keep it private and secure. Except as otherwise described in this notice, we restrict access to nonpublic personal information about you to employees of our gym who must use that information to provide services to you. Thank you for allowing us to serve all your fitness needs. We value your business and are committed to protecting you privacy.
A. Types of Information We Collect
We collect information you provide when you inquire about our services, complete intake forms or waivers, or manage your membership. This may include your name, contact details, billing information, emergency contact details, fitness goals, injury history, and medical conditions you disclose.
We use this information to administer memberships, process payments, communicate with you, and support the delivery of our fitness services.
We offer an optional health-history questionnaire to help coaches adapt training to your needs and limitations. If you choose to complete it, we collect the health information you provide through an online form and store your responses electronically. Access is limited to staff who need the information to support your training. Health questionnaire responses remain stored electronically; we do not currently have an automatic deletion schedule.
B. Using and Disclosing Your Personal Information
Personal Information will be used for the following primary purpose:
- We use member and inquiry contact information for billing, scheduling, membership support, and general gym updates.
FOUNDATION may employ other companies or individuals to assist us in providing our services, or to provide certain services such as providing marketing assistance or consulting services. These third parties may have access to information needed to perform their function but cannot use that information for other purposes.
Service Providers
We use third-party providers to support membership management, billing, communications, social media activities, and online forms and response storage. These providers may receive, store, or process personal information as needed to provide those services.
Information you share through social media platforms is also subject to those platforms’ privacy policies. Providers we previously used may continue to hold historical contact information until those records are deleted.
Photos and Videos
We collect permission to use member photos and videos through the waiver completed during onboarding. We may share these photos and videos through our communication channels, including social media, within the scope of that permission.
Access To Personal Information
You may request access to, correction of, or deletion of your personal information by emailing info@foundationgym.us. We may need to verify your identity before processing your request.
We will review and respond to requests in accordance with applicable law. If we cannot fulfill part of a request, we will explain the reason and any applicable options for further review.
Storage and Security of Personal Information
We maintain waivers and health questionnaires electronically and no longer retain paper copies of those records. Access to these records is limited to staff who need them to perform their work.

FOUNDATION and Links to Other Websites
FOUNDATION may provide links to web sites outside of the FOUNDATION site. These linked sites are not under the control of FOUNDATION, and FOUNDATION is not responsible for the conduct of companies linked to the FOUNDATION web site, nor for the performance or otherwise of any content and/or software contained in such external websites.
